License: CA state license required; pending licenses accepted, but must be licensed upon start date. Certifications: None specified. Must-Have: - Previous charge experience - Knowledge of InterQual criteria, DRG, and CMS regulations - Experience with care coordination and discharge planning - Ability to organize and participate in family meetings - Experience with assessing patients' needs - Competency in arranging DME, SNF, and ambulance transport - Proficient in Microsoft applications - Must be able to perform tasks independently - Experience managing patients with mental health history or in crisis Description: This position is for a case manager specializing in acute care. The role involves performing care coordination and discharge planning for patients. Hospital experience is essential, and candidates without it will be redirected.
